Healthy Living with Type 2 Diabetes

Scope

Providing education to those diagnosed with type 2 diabetes and living within the Torbay and South Devon area as per NICE guidance. The education programme is Quality Institute for Self-Management Education and Training (QISMET) certified.

The healthy living programme for people with type 2 diabetes provides information and advice to increase the knowledge, skills, and confidence in self-management of type 2 diabetes. The programme will make people aware of the potential consequences of diabetes, enabling them to make healthy lifestyle changes and be informed about the health checks required.

The programme provides people with a choice over how they access education, and includes the following options:

  • Group sessions (virtual or face-to-face)
  • A digital (App-based) programme
  • Referral to the national online diabetes education programme,

Opportunity to attend ongoing education is available to all upon completion of the programme.

Out of Scope
  • People under the age of 18 years old
  • People with a diagnosis of Type 1 Diabetes or in the pre-diabetes range
  • People who are pregnant
  • People outside of Torbay and South Devon NHS Foundation Trust catchment area

Patients who are acutely unwell with osmotic symptoms should not be referred and this referral should be delayed.

HbA1C over 48.

After being referred to the programme patients will be contacted via phone or email. They will be asked questions to help them choose which course is best for them, and if there is additional support that can help them to get the most out of the course. They will then be directed to their course of choice.

We will attempt contact 3 times, over email, phone call, or letter, if we are not able to make contact, we will close the referral. Regardless of engagement level when discharging patients, we will send an update to the referrer with the outcome of the referral.

    • National online course that people can work through at their own pace available through a web browser
  • One to One support
    • Those who are unable to attend a face-to-face course and unable to use digital options are offered the opportunity to receive a posted resource pack and telephone appointment

What patients can expect

  • Information on a range of topics including treatment and care for type 2 diabetes, healthy eating, improving activity levels and emotional wellbeing.
  • Help to understand the potential consequences of living with diabetes.
  • Knowledge and support to increase confidence to make healthy lifestyle changes.
  • Patients will be prepared for any health checks they might need.

In group sessions, patients can expect

  • Time to discuss questions or concerns with health professionals.
  • Opportunity to meet others who have recently been diagnosed with type 2 diabetes (they are free to bring a partner/spouse/friend with them).

To attend online sessions patients will need.

  • Internet connection
  • Microphone
  • Camera

Session 1 is designed to support patients in getting used to the technology and additional support is available if needed.

If patients do not attend, we will contact them to prompt them to rebook, if no contact we will then discharge them.

Once patients have been discharged following either completing the course, declining the course, or uncontactable, the referrer will be notified of the outcome of the referral so that the outcome can be SNOMED coded.

Referral Criteria

Please note referral criteria are applicable in this referral and referrals may be returned if this information is not contained within the information you send.

  • Age 18+
  • HbA1c = 48+ mmol/mol (or confirmed diagnosis of type 2 diabetes and HbA1C is lower than 48 due to medication)
  • Not pregnant
  • Not Type 1 Diabetes
  • GP surgery within Torbay and South Devon
