Type 2 Diabetes Structured Education Programme - Sentinel Healthcare

Scope

Providing Type 2 diabetes education to adults that are newly diagnosed (<24 months) and living within the Western Locality area as per NICE guidance. The education programme is Quality Institute for Self-Management Education and Training (QISMET) accredited.

The programme provides an initial individual onboarding telephone consultation, followed by two interactive group sessions of education (face to face or digital options available). Each session is 2.5 hours.

The programme aims to provide the knowledge and skills that will increase confidence in terms of self-management and control of their condition. To make them aware of the potential consequences of diabetes and enable them to have confidence to make healthy lifestyle changes and be informed about the health checks required.

Out of Scope

  • People under under 18 years of age
  • People with a diagnosis of Type 1 diagnosis or in the pre-diabetes range.
  • People that are pregnant.
  • People registered with a GP practice outside of the Western locality in Devon

On referral patients will be invited to an initial onboarding telephone appointment that aims to motivate attendance at the education sessions and to undertake baseline assessments of the individual learning needs and level of knowledge. The patient will be offered the choice of attending face to face group sessions or the digital options available. We will attempt to contact the patient two more times before we discharge them. Regardless of engagement level when discharging patients, we will notify the referrer of the outcome of the referral.

  • Face to face courses involve two sessions lasting 2.50 hours are led by a Diabetes Nurse Educator and a Dietitian Educator. The sessions take place at various locations across the Plymouth city area and the South Hams outlying areas. There are daytime weekday and weekend options. Patients will receive a Workbook at the first session, which the patient can use to record information and use a learning resource at home.
  • Remote courses involve two Zoom webinars lasting 2.50 hours and are led by a Diabetes Nurse Educator and a Dietitian Educator. Patients will be emailed a digital version of the Workbook prior to the first webinar, which they can use to record information and use as a learning resource. Sessions take place during weekdays daytimes and evenings.
  • MyMHealth Diabetes App. Patients also have the option to sign up to using the MyMHealth Diabetes app. myDiabetes is a fully-featured, NICE-aligned platform that provides expert advice, structured education, nutritional guidance and exercise programs, With blood glucose monitors, linked via Bluetooth, and the ability to build a personalised set of targets, myDiabetes helps people control their condition. Clinicians also receive access to remote care, patient management, reports and monitoring tools that makes caring for patients faster, easier, cost-effective and ultimately more rewarding. What patients can expect at the sessions
  • Information and videos on a range of topics including treatment and care for Type 2 diabetes, healthy eating and improving activity levels.
  • Help to understand the potential consequences of living with diabetes.
  • Knowledge and support to increase confidence to make health lifestyle changes.
  • Patients will be prepared for any health checks they might need.
  • Workbook activities to complete.
  • Time to discuss questions or concerns with health professionals.
  • Meet others who have recently been diagnosed with Type 2 diabetes (they are also welcome to bring a partner/spouse/carer with them).

If patients do not attend a booked course we will attempt to contact them by telephone to rebook. If no contact, then we will send a letter/email and if there is no response within 7 working days, the patient will be discharged.

Once patients have been discharged following either completion of the course or declining to attend the course, the referrer will be notified of the outcome of the referral so that the outcome can be SNOMED coded.

Please note referral criteria are applicable in this referral and referrals may be returned if this information is not contained within the information you send.

  • Age 18+
  • HbA1C = 48+ mmol/mol (HbA1C within the last 24 months)
  • Not pregnant
  • Not Type 1 diabetes
  • GP within the Western locality.
