NHS Digital Weight Management Programme

Key Messages

The new NHS Digital Weight Management Programme (DWMP) offers online access to tier 2 weight management services for those living with obesity plus diabetes or hypertension or both.

With three levels of support and a choice of Providers, it is designed to offer service users a personalised level of intervention to support them to manage their weight, improve quality of life and improve longer term health outcomes. Further information about the programme can be found here.

The programme will work alongside, and not replace, existing weight management services.

Scope

  • patients that are aged 18 years and over AND have a BMI greater than 30kg/m² (adjusted to 27.5kg/m² for people from Black, Asian, and ethnic minority backgrounds) recorded within the last 24 months AND have a diagnosis of diabetes or hypertension or both
  • NHS staff with a BMI over 30 (27.5 for BAME groups)

Out of Scope

  • In Devon, there is currently no weight management service for people below 18:
    • For under 18s consider a referral to paediatrics or a dietician
  • Pregnant women
  • Patients with an active eating disorder
  • Patients who have had bariatric surgery in the last two years
  • Patients whom the referring practitioner feels a weight management programme is considered to pose greater risk of harm than benefit are not eligible for the programme.

Practices can identify eligible participants through practice system searches. A tool that facilitates this search for the eligible population is provided as part of the NHS Digital Weight Management Programme GP Template Site.

Referral Criteria

  • patients that are aged 18 years and over AND have a BMI greater than 30kg/m² (adjusted to 27.5kg/m² for people from Black, Asian, and ethnic minority backgrounds) recorded within the last 24 months AND have a diagnosis of diabetes or hypertension or both.
  • NHS staff with a BMI over 30 (27.5 for BAME groups).

Referral Instructions

  • Referral into the NHS Digital Weight Management Programme will be initiated by General Practice via the NHS Digital e-Referral system (e-RS) by inputting MIDLANDS & LANCASHIRE – PARTNERING NHSE/I via the organisation search alternatively you can search using the postcode – B70 9LD.
    • Please select Specialty Dietetics and Clinic Type Weight Management.

Please note NHS Midlands and Lancashire CSU hosts this National NHS Digital Weight Management Programme on behalf of NHSE/I and is accessible by ALL GP practices in England

Templates are available for EMIS and System1. Currently, the programme does not allow referral from secondary care or self-refer.

Patients living with obesity without the comorbidities required to make them eligible for the NHS Digital Weight Management Programme, should be considered for referral into existing weight management services.
