Improving Lives Plymouth Patient Programme

Long-term Condition Self-Management Programme

GPs and patients can refer to Improving Lives Plymouth for patient support with any long-term physical or low level mental health condition. The service helps people to better manage their condition and to achieve a better quality of life.

The free service is for patients living in the Plymouth area aged 18 years or over who has any long term physical of low level; mental health condition. This service is an accredited chronic disease management programme aiming to find new ways of managing conditions and help people to lead to an improved quality of life.
