5.1.4 Aminoglycosides

Gentamicin
  • Solution for injection 20mg/2ml, 40mg/2ml, 80mg/2ml
  • Intrathecal solution for injection 5mg/ml

Notes

  1. For patients with Cystic Fibrosis (CF): treatment should only be prescribed by secondary care
  2. Blood level monitoring is mandatory for continued therapy.
  3. Gentamicin may be prescribed for nebulisation under recommendation from an appropriate secondary care consultant.
  4. It should be noted that nebulisation of gentamicin is an established but unlicensed use, see here for information on unlicensed/ off-label use.
Tobramycin
  • Solution for injection vials 80mg/2ml
  • Nebuliser liquid ampoules 60mg/ml (300mg in 5ml), 75mg/ml (300mg in 4ml)
  • Inhalation powder capsules with device 28mg

Notes

  1. For patients with Cystic Fibrosis (CF): treatment should only be prescribed by secondary care
  2. NICE TA276: Tobramycin dry powder for inhalation (DPI) (TOBI Podhaler) is recommended as an option for treating chronic pulmonary infection caused by Pseudomonas aeruginosa in people with cystic fibrosis (March 2013), only if:
    1. nebulised tobramycin is considered an appropriate treatment, that is, when colistimethate sodium is contraindicated, is not tolerated or has not produced an adequate clinical response, and
    2. the manufacturer provides it with the discount agreed as part of the patient access scheme to primary, secondary and tertiary care in the NHS
